Why Advocate Health called 15,000 patients in 12 days with AI

Advocate Health used conversational AI from Hippocratic AI to call more than 15,000 hypertension patients who had not recently checked their blood pressure. During the 12-day effort, the AI guided patients to take readings at home, identified about 1,200 with normal levels, and escalated hundreds of others to human nurses when readings were high or additional health concerns were raised. The outreach also uncovered unrelated health issues, creating new opportunities for follow-up care and improving engagement compared to traditional call methods.

The system was designed with clinician input and included clear disclosures that patients were speaking with AI, along with options to connect to a human when needed. Advocate Health plans to expand the use of Hippocratic AI’s agents to support more continuous patient follow-up, improve efficiency, and extend similar approaches to other conditions and administrative tasks. Early results are being used to guide staffing and scaling decisions as the health system prepares for broader deployment.
