A computing system can obtain health record data of a user from a number of health record sources, and generate, on a computing device of the user, an interactive user interface comprising prepopulated data based on the health record data. Based on interactions by the user on the interactive user interface, the system can receive input data from the computing device of the user, the input data corresponding to health-related information provided by the user. Based on at least one of the health record data or the input data, the system can generate a set of health outcome predictions for the user, and based on the set of health outcome predictions, the input data, and the health record data, the system can generate a set of health care plan rankings for the user.

BACKGROUND
Health record access is typically provided through individual health care providers, healthcare organizations, insurance companies, pharmacies, pharmacy benefit managers, and others. It is nearly impossible for any individual to construct and access a central record repository from these disparate sources. Health care services are provided by health care providers through health care plans, which can vary in coverage, cost, deductibles, quality, and copays. Individuals are typically tasked with obtaining their own health care plans.
Call sessions between users and call agents for health care services may require extensive information gathering and verification of personal health and medical history information to enable a system to determine an optimal health care plan for the user. Such information gathering and verification can correspond to the user’s personal information and medical history (e.g., past medical conditions, current medical conditions, past and current prescriptions, diagnoses, and the like).
